Medical services are provided only to patients located in states where Dr.HanahPolotsky is licensed to practice medicine. You are responsible for ensuring that you are physically located in an eligible state at the time of any telemedicine encounter.  Prescription decisions are made only after appropriate medical evaluation and when clinically indicated.

Telemedicine Informed Consent Disclosure (Colorado C.R.S. § 12-240-107)

Telemedicine services involve the delivery of healthcare using electronic communications. As with any medical service, there are potential risks, including technical failures, limitations on physical examination, and privacy risks inherent in electronic transmission. Medical services are provided through telemedicine using secure video technology. Telemedicine has limitations compared to in-person care and may not be appropriate for all medical conditions. By scheduling a visit, patients acknowledge these limitations and consent to telemedicine care.

You have the right to withhold or withdraw consent at any time. Telemedicine services are provided only to patients physically located in a state where Dr. Polotsky holds an active medical license at the time of the encounter, including New York, New Jersey, and Colorado.

Scheduling Disclaimer

Scheduling a consultation does not establish a physician–patient relationship. A physician–patient relationship is established only after a formal consultation and mutual agreement to provide care.

No Surprises Act — Good Faith Estimate Notice (45 C.F.R. § 149.610)

You have the right to receive a 'Good Faith Estimate' explaining how much your medical care will cost. Under the No Surprises Act, healthcare providers must give patients who don't have insurance, or who are not using insurance, an estimate of the expected charges for medical services before scheduling. You can ask for a Good Faith Estimate before any service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Scope of Practice / Dual-Entity Disambiguation

Leadership consulting, coaching, and educational services offered through OpExMD Consulting, LLC do not constitute medical services and do not establish a physician-patient relationship. These services are distinct from clinical medical services provided through OpExMD, PLLC. Performance Strategy and Leadership Optimization services provided within the Optimization Program are intended for professional development and executive performance. While informed by your physiological profile, these sessions are not medical treatments for psychiatric disorders and do not constitute or replace professional psychotherapy or mental health counseling. Engagement in performance coaching does not replace the need for a primary care physician or a licensed mental health professional for non-endocrine related concerns.

By submitting your email address, you consent to receive educational communications from OpExMD. You may unsubscribe at any time. We do not sell or share your email address. Subscribing does not create a physician-patient relationship.

Direct-Pay Practice

OpExMD is a direct-pay medical practice and does not bill or accept insurance. Fees, policies, and program structures are outlined separately in the patient agreements. Program fees do not include labs, imaging, or prescriptions, which are billed separately by those entities.

Notice Regarding Medicare & Medicaid Beneficiaries

OpExMD is a specialist direct-pay practice and does not participate in the Medicare or Medicaid programs on this Website.. Because Dr. Polotsky is an enrolled Medicare provider in other clinical settings, federal law requires that she bill Medicare for all covered services provided to Medicare beneficiaries. As OpExMD is not structured to support Medicare billing, we are unable to accept patients who are currently enrolled in Medicare or Medicaid (including Health First Colorado, NY Medicaid, and NJ FamilyCare). By using this Website or requesting a consultation, you represent and attest that you are not a Medicare or Medicaid beneficiary and that you do not have active coverage under these programs. We cannot accept out-of-pocket payments from Medicaid beneficiaries for services that are otherwise covered by the state medical assistance program. If your insurance status changes, you are required to notify us immediately.
